First TV show I had self-insertion fantasies about:
Captain Bucky O'Hare, when I was three. Also Superted. I don't think I really self-inserted into many fandoms as I got older, though I did have a lot of 'a bit like me but it's an original character honest' fantasies.

I'm leaning towards apiphile's bandom stuff, though I think some of the BtVS writers I used to follow dabbled in popslash, so I may have read that earlier.

First fanfiction I read that made me think, YES, this is exactly the kind of fanfiction I'd like to write.
There was a Spike one by someone calld Zero, which was essentially a short horror story about picking up hitchhikers (and being picked up). Been looking for it a long time since.

Probably Buffy/Spike.

First OTP:
Buffy/Spike, though Jaina/Kyp (Star Wars EU) was the first one that had me arguing fervently in favour of it online.

Oh, my self-inserts weren't direct self-inserts either, they were all hideous Mary-Sues. But I kind of view them as self-inserts because they were blatantly ways for me to be in the story.
